Surveys help you gather feedback from your audience. This guide walks you through creating a multi-page survey using different question types including ratings, multiple choice, and open-ended questions.
What you’ll build
Section titled “What you’ll build”By the end of this guide, you’ll have a survey with:
- Welcome page with introduction
- Multiple pages for organized questions
- Various question types (rating, multiple choice, open-ended)
- Navigation between pages
- Thank you page with confirmation
Time to complete: 15-20 minutes
Planning your survey
Section titled “Planning your survey”Before building, consider:
- Goal: What do you want to learn?
- Audience: Who will take this survey?
- Length: How many questions are reasonable?
- Question types: What formats best capture the data you need?
For this guide, we’ll create a customer satisfaction survey.
Step 1: Create a new form
Section titled “Step 1: Create a new form”-
Log in to your Fomr dashboard

Click the New form button

You’ll see an empty form canvas
Step 2: Create the welcome page
Section titled “Step 2: Create the welcome page”Start with an introduction to set expectations:

Drag a Text component onto the canvas

Set the content to introduce your survey:

Set the variant to Heading for the title portion

Add Page Actions with a “Start Survey” or “Next” button
Step 3: Add page 2 - overall satisfaction
Section titled “Step 3: Add page 2 - overall satisfaction”Create a new page for satisfaction questions:

Add a new page to your form (click the + to add a page)

Drag a Text component and add “Overall Experience” as a section header

Add a Rating component:
- Label: “How satisfied are you with our product overall?”
- Max stars: 5
- Required: Yes

Add a Linear Scale component:
- Label: “How likely are you to recommend us to a friend or colleague?”
- Max value: 10
- Low label: “Not at all likely”
- High label: “Extremely likely”
- Required: Yes

Step 4: Add page 3 - specific feedback
Section titled “Step 4: Add page 3 - specific feedback”Add questions about specific aspects:

Add a Text component: “Product Feedback”

Add a Multi Choice component:
- Label: “Which feature do you use most often?”
- Options: List your main features
- Required: Yes

Add a Checkboxes component:
- Label: “What do you like most about our product? (Select all that apply)”
- Options: Ease of use, Features, Price, Support, Reliability, etc.
- Required: No

Add a Matrix component:
- Label: “Please rate the following aspects:”
- Rows: Quality, Value, Support, Ease of use
- Columns: Poor, Fair, Good, Very Good, Excellent
- Required: Yes

Section titled “Step 5: Add page 4 - open-ended feedback”Give respondents space to share detailed thoughts:

Add a Long Answer component:
- Label: “What could we do to improve your experience?”
- Description: “Your feedback helps us prioritize improvements”
- Required: No

- Label: “Is there anything else you’d like to share?”
- Required: No

Step 6: Add the thank you page
Section titled “Step 6: Add the thank you page”Create a completion screen:

Add the Thank You component after the last page

Set the title: “Thank you for your feedback!”

Set the message: “Your responses have been recorded. We truly appreciate you taking the time to help us improve.”

Choose the Animated icon

Optionally toggle Confetti on
Step 7: Review your survey structure
Section titled “Step 7: Review your survey structure”Your survey should now have this structure:

Step 8: Customize the design
Section titled “Step 8: Customize the design”Make your survey visually appealing:

Click the Design tab

Choose a theme that matches your brand

Set your primary color

Consider adding a subtle background

Adjust form width to Medium or Large
Step 9: Configure settings
Section titled “Step 9: Configure settings”-
Click the Settings tab

Enable Email notifications to get alerted on new responses

Consider enabling Auto-jump for a smoother experience (optional)
Step 10: Preview and test
Section titled “Step 10: Preview and test”-
Click Preview to test your survey

Go through every page as a respondent would

Test all question types

Verify the thank you page displays correctly

Check that navigation works smoothly
Step 11: Publish and share
Section titled “Step 11: Publish and share”-
Click Publish to make your survey live

Click Share to get your survey link

Distribute via email, social media, or embed on your website
Survey best practices
Section titled “Survey best practices”Keep it focused. Stick to questions that directly support your survey goal.
Respect time. Tell respondents how long the survey takes and stick to it.
Mix question types. Use a variety of formats to keep respondents engaged.
Make most questions optional. Required questions can frustrate respondents and lead to abandonment.
Start easy. Begin with simple questions before asking for detailed feedback.
End positively. Thank respondents and let them know their feedback matters.
Test thoroughly. Take your own survey multiple times before sharing.
Analyzing results
Section titled “Analyzing results”After collecting responses:
- Review individual responses in your dashboard
- Look for patterns in ratings and selections
- Read open-ended responses for qualitative insights
- Calculate your NPS score (% Promoters - % Detractors)
- Export data for deeper analysis if needed
